CASE MANAGER

Description: The key functional role of the case manager is coordination or arrangement of services (formal and informal) across a variety of service setting. The case manager strives to ensure that clients with complex needs receive timely, coordinated services and that resources are utilized in order to maintain an individual’s ability to function independently in a community of their choice as long as practical. In addition, the case manager will provide services in a manner in which the client maintains self-sufficiency and promotes self-empowerment.

Case management includes: Assessing and reassessing the need for client services; negotiating and advocating for the development and delivery of needed services; direction for clients seeking access to services; educating consumers and professionals around HIV disease and resources available to support affected individuals and family members.

Major Responsibilities:

1. Provide comprehensive services to clients in a timely, efficient and professional manner according to the State of Wisconsin Practice Standards and Administrative Guidelines for HIV Related Case Management.

The case manager will perform assessments as assigned by the Director of Life Care Services.
Within seven (7) working days following initial assessment, develop a service-plan with measurable time-limited goals.
Update the service-plan quarterly, at minimum, and more often if there is a status change in the client’s situation.
Maintain contact with the client monthly or quarterly (frequency specified the Wisconsin Acuity Assessment) for the purpose of service delivery, monitoring need for new services, and gauging client satisfaction.
Document all contact with client, client support system, providers and other participants in the client’s file (progress notes) within five (5) working days of contact.
Annually, perform reassessment for each client and develop a new service-plan subsequent to reassessment.
Maximize client’s use of available natural supports, including family members as the client defines them.
Coordinate services among multiple providers to avoid duplication of services.
Document discharge of clients from case management services within timeframes specified by state Practice Standards.

2. In cooperation with the Director of Life Care Services and Case Management Program Assistant, annually survey clients regarding satisfaction with case management services.

3. Maintain relationship with community providers as requested and identify provider training needs.

4. Participate as required in regular meetings. Develop and maintain good working relationships with staff, board members, consumers and volunteers.

Support agency mission
Attend weekly Case Management Team meetings and monthly All-Staff meetings
Represent agency in a positive manner to community partners as appropriate
Communicate regularly with other staff on program changes, needs, successes, etc.
Adhere to all policies and procedures set forth by the agency

5. Complete and submit in a timely fashion all necessary information for monthly, quarterly and annual reports.

6. Travel as required for the provision of case management services.

7. Other duties as assigned by supervisor.

Qualifications:

1. Minimum Bachelor’s degree in Social Work or another related field. Master’s degree preferred.
2. 1-2 years employment experience (more is preferred) in human services field, with special consideration given to those whose combined work and volunteer experiences include a focus on substance use, mental health, or chronic manageable illness, especially HIV/AIDS.
3. Oral and written communication skills.
4. Computer experience, preferably including Lotus-based applications
5. Ability to work effectively with individuals of diverse backgrounds
